HFM Shared Services 11.1.2.4 Access Control Report Template Missing/Report Does Not Run

When going into HFM Shared Services in 11.1.2.4 to run the Access Control Report, the report template is blank.  Also, I have tried to run the report and am not getting any results. 

Following these steps from the Admin guide and I am not able to get the report to run: 

Has anyone had this issue?
