Siebel - Oracle upgrade from 11.2.0.x to 12.2.0.1

Team,

We are using Siebel IP 15.12 and Oracle 11.2.0, Now we are planning to upgrade Oracle to 12.2.0.1, so want to know what are the things need to considered before pre upgrade and what to check after post upgrade. Apart from this, i have query regarding whether its supports IP 15.12 because i heard that we have to install patch IP 15.18. In case of patch, from where can we get.
