Creating an eval with Free Text sections only

We have a request to create a new template for ePerformance that would allow free text broken into 4 categories in which the EE would complete their self-eval that would flow to the manager to then complete the same 4 free text categories before routing to the next level.

1. Accomplishments

2. Strengths

3. Development Needs

4. Career Interests

I found the option to use "Free Form Items", but it appears that is just as an add on the user chooses while completing the remainder of the eval. Does anyone know if there is a way (delivered or custom) to create something along these lines? Or has anyone implemented something similar?
