Set up of Vermont Health Care contribution tax

Effective March 31, 2020 and through December 31, 2020, the HCC is $184.42 (up from $167.02 for the same period in 2019 and $163.20 for the same period in 2018) per uncovered full-time equivalent (FTE) above the four FTE exemptions.  

I am not sure about the set up of the Vermont Health Care contribution in PeopleSoft HCM version 9.0.  

And I don't see anything or anywhere it is related to the state tax setup for VT.  The form looks like something Randstad would file and not an employee. So, I'm guessing it might be an employer contribution and not an employee contribution?
