WLS Upgrade (12.1.2 to 12.2.1.3)

Hi community.

Is there any specific doc to migrate WLS 12.1.2 to 12.2.1.3? I have the official guide (https://docs.oracle.com/middleware/12213/wls/WLUPG/intro.htm#WLUPG107) but it's very high level and it's not quite detailed enough.

Should we to install the new version from zero or could we to do only the upgrade? Could you share with me the neeeded binary links?

And I would like to add one more question, we have a Weblogic 12.2.1.2 and detected CVE-2018-3191 vulnerability. I know it was solved on CPU Oct-2018, I would like to know If it's possible to apply this CPU Oct-2018 on 12.2.1.2 or we will need to upgrade first to 12.2.1.3
