adop phase cleanup or prepare give error "Cannot connect to database"

After 12.1.3 to 12.2.9 upgrade, the application is running without issue.  I applied the first patch after upgrade, finished the phase prepare, apply, finalize, and cutover.  The run edition changed from fs1 to fs2.  Again the application is running fine.

Now, I want to do another patch, so I use adop cleanup (or prepare), but give this error:
