Ad hoc insert '+' sign is not available on approval page

Hello,

We need to insert an ad hoc approver/reviewer at the time of approval but '+' sign of Ad hoc insert is not available. On the requisition or PO page, Ad hoc insert is available but we need to insert an

ad hoc approver/reviewer at the time of approval.

Need help.
