CafeOne Second Form Not Loading for End Users

I ran through all the steps of creating a CafeOne - created the publication list, enabled the base form, created the layout and then copied my user overrides to the roles that needed to see the layout.

When I open the base form, the second form pops up just fine on my account, but when I go to one of my end users, the layout shows up in the dropdown at the top, but when it's selected, nothing comes up, not even a blank form, just the original form.

Does anyone have any idea why this would be happening?
