For all 12.1.0.2 installs and upgrades on Linux x86-64, please be aware of the issue discussed in this My Oracle Support document:

Note that the best treatment for this issue is PREVENTION!

Workaround

Before starting runInstaller when installing of or upgrading to 12.1.0.2 Grid Infrastructure (GI), set SRVM_USE_RACTRANS to true.

On ksh or bsh, issue "export SRVM_USE_RACTRANS=true"

On csh, issue "setenv SRVM_USE_RACTRANS true"

After SRVM_USE_RACTRANS is set to true, execute runInstaller to start the installation or upgrade.
