Planning UI Issues After Upgrading to EPM 11.1.2.4

We have seen numerous issues where the in-place upgrade to EPM 11.1.2.4 failed to update the underlying ADF patches, leading to various UI issues in Planning after the upgrade.
I created the following Note to show you how to check for ADF patches and manually install them:
