Why is the concurrent manager showing the request set name twice?

When I submit my request set it is showing the name twice. I believe normally it would show: NAME(request set). Mine is showing NAME(request set NAME) repeating the set name after "request set" in the parenthesis. Does anyone know what would cause this or how to fix it?
