Why is the name of my request set repeated when it displays?

I am using EBS release 12.1.3. I created a request set and defined it manually. When it is submitted, it is repeating the set name in the parenthesis after "Report Set". Instead of showing NAME(Report Set) it is showing NAME(Report Set NAME). It is only doing this for one request set. Does anyone have any ideas of why it would display this way or how to fix it?
