We are in R12.1.3, want to capture Readings for Operations involved in PM for Asset Route

I have 20 Assets in the Asset Route and want to capture some temperature & pressure Readings for all the Assets and WO is created for Asset Route. We are using R12.1.3 . I defined a QP to capture the readings but in OAF Page it shows one row and then user needs to add an other row ...like that

If at least any of the below options if i get is well & good.

1) While entering QP at WO Completion the Quality Collection Plan Results Form should open as its easy to select the Individual Asset from LOV and enter multiple records

2) Any attachment is mandatory based on Asset (Asset Route) or Activity at WO Completion so that i can add an excel attachment to capture the values
