Pending transactions with "An error occurred while relieving reservations."

Hi Members,

We are currently in R12.2.7 and are facing an issue with pending transactions which are in error with message "An error occurred while relieving reservations.". We have WMS enabled for the org and upon analyzing we found that LPN123 which is picked is then transferred to LPN 456 after picking and then ship confirmed. When we check the mtl_onhand_quantities_table, it shows LPN456 in the parent LPN id for LPN 123 and when we check the material workbench, it shows LPN123 in Staging subinventory with 0 Total Quantity.

Not sure what cause the Total Quantity for the LPN to be 0. Does anyone have any suggestions on what could be the cause?
