how to fetch details of information templates from smart form requisition on PR PDF report-SSP Cloud

Hello All,

We have recently configured Information templates on our each smart forms in Self service procurement cloud service. The challenge is we are unable to find details entered by the requester in information templates on smart form requisition. We need to fetch it on PR PDF report. Can anyone provide us the query or a solution to achieve this?
