OAB 12.2 - ACA 1094C Download Part II missing months and throwing errors

Has anyone in 12.2 gotten the 2020 1094C download for part II working after patches 31630959 and 32434879? One of my staff can run it and save, except it's missing Box 16 months May-June and September-December. New zip code fields show up.

When I try to run it, I get a VB error '91' Object variable or With block variable net set. My Excel Trust Center has trust the VB project and Enable all macros, and it still won't work. I don't see any bug postings on Support.
