Having error in SQLT installation in 19c PDB(ORA-06512: at "SYS.UTL_FILE", line 41)

Tried installing SQLT disanostic tools:

# sqlplus / as sysdba

SQL> START sqcreate.sql

I have uploaded the installation errror logs with the same.

We are seeing following error:
