ORA-03113: end-of-file on communication channel

Dear Experts,

There is a 12.2 RAC Enterprise edition ruining on Exadata X7 platform.

Every time end user tries to renew its expired password it gets ORA-03113 error and process is crashed.

Do u have any idea what could be causing this error?
