iRecruitment - Applicant Assessment questions and answers - report?

Hi, does a report already exist or has anyone written one which provides Applicant Assessment questions and answers?

I have been asked to provide:

Vacancy Name - IRC & job title

Manager

Function

Grade/Band

Recruiter name

Recruitment Process

Candidate Name

Candidate email address

Applicant Assessment (Screening questions) – we would like the questions and answers to show on the report – No word limit if possible.

Any help would be greatly appreciated.
