Personal form issues with applications having on processing options

I am trying to create a default personal form for P4006. I created it, but it ended up creating it with version CPG0011, but there is no CPG0011 version. in fact there are no versions.

So I tried to create a version, but I cannot do that as there are no processing options.

How do I handle personal forms on applications with no processing options.
