EAM - Zero actual duration WOs | Actual start & end times same as Scheduled start & end times?

Hello,

Please see the attached screenshot of our WO. We've routine & emergency work order types completed by our plant users that has Actual start & end times same as Scheduled start & end times - upto the second?.. While the applied resource unit on those WOs is ~4hrs.

Question is, we've EAM: Actual Work Completion Date profile set to "system date". How the actual and scheduled times be the same? One observation in looking at those WO's is that "Firm" flag is on? What is the cause of this?
