How to add a record into table csi_item_instances.

Details as below:

  1. Oracle EBS version R12.1.3
  2. item not picked 'Track in Installed Base' Flag.
  3. Order shipped and closed.
  4. Not able to find any record in the csi_item_instances table and error processing.
  5. need to manually add the record for Service request creation.
  6. Will enable this option.

Any suggestion?
