Smart Form

A former colleague created a Smart Form that has multiple options to pulldown under the "Item Type" field, see attachment. I want to delete all pulldown options BUT services billed by amount. The colleague who created this Smart Form left the agency hastily without sharing the knowledge of how to remove these other pulldown options. Any help would be appreciated, please. Thank you.
