The User List "Position Management" is picking wrong person for approvals

Hello All,

I am working on FORMS and I have set the following approval process:

The problem is, the approver user list at Step no 3 is not picking up the right person for the approval, it is showing a name from last / final approving authority. Whereas in step 03 there should be name of employees immediate manager (to whom he is reporting).


This process used to work fine, but it is malfunctioning now, any idea how can I troubleshoot this?
