FRM-92101: there was a failure in the forms server during startup. this could happen due to invalid

Hi 

As I m creating Mass allocation formular.

path: GL Supper User> Journal > Define > Allocation

step 1 > enter batch name

Step 2> Click formular button 

Step 3> Enter name, Category, and currency 

Step 4 click to account to enter Chart of account combination then Error appear which the message as :

FRM-92101: there was a failure in the forms server during startup. this could happen due to invalid configuration 

I include screenshots to give a better describe this issue

Could you give us any advice to solve this issue
