When access Receiving Transaction Status Summary Form, I meet one error

Oracle Applications : 12.2.8

The error message "You are setup as a worker. To access this form you need to be a worker ".

But in fact, we can't find any problem on User and Employee information.

So need help on this, why this error message generated.

Another question:

When I login Oracle and open form, I meet error "FRM-40735: When-New-Form-Instance trigger raised unhandled exception ORA-06502 ".

Have a try that clean Browser and JAVA Cache, the error still happens. So need help, too.
