F43121 not updated

I'm helping Finance reconcile a VERY old problem. I have found what initiated the problem, but I can't say I found the root cause.

A PO receipt hit the GL (RNV account) and it hit the F4111 Item Ledger. But there is no record of it in the F43121. Have you ever seen something like this happen on the 9.2 release?

This is so old, and I would not be able to duplicate it, so I feel like opening a case is a lost cause. I was just wondering if any of you have seen something like this happen on newer release (I know this type of thing could happen in old, old, old releases of JDE but not on 9.2)
