how to create ALert for SQL query

hi

what are different ways available to generate Alert for a particular SQL query (not generic) if it exceeds a particular elapsed time or logical or physical reads.

this is not for all sqls. Is there anyway we can monitor a particular SQL and create incident for the same for its different metrics warnings and critical thresholds.
