SQL query to get DB name and Patch level with date when last patch applied

Hi

I am looking for some SQL which can be run against OEM repository to fetch very specific details like DB name, DB Version, Patch Version and Last Patch Applied. This is required for all the database targets we have in the estate.

Does anyone please help me with the SQL.
