COBRA events not qualified

I am trying to turn on COBRA. I have the installation table setup done, the program plan type checkbox done, the action reason codes done. I am testing a Termination. the employee has coverage in a qualified plan, and has had coverage for several years. the employee shows in the COBRA Trigger activity, but when I run PSPCOBRA the employee is Not Qualified. I've tried several people with the same results. the event date is after the setup table dates. any ideas?
