EEO-1 Errors uploading PER016

This is the first year we have attempted to import the file to the EEO site. In years past, we ran the delivered report and manually keyed the values. We received thousands of errors indicating "Required field not provided' for address, NAIC, report filed last year , has 100 etc. However these values are indeed populated. We checked our configuration on the establishment and location tables and all looks fine. Has anyone else experienced format issues with loading the delivered report to EEO ?
