BI Publisher W-2 - No Filing Instructions in PDF

Hi, I wondering if anyone can offer some assistance with the BI Publisher printing process.

When I execute Create W-2 PDF Forms, the PDF file is created, but when I view the PDF in Report Manager, the filing instructions are not in the PDF with each W-2.  I have the attachment configured in Tax Form BI Publisher Options and can view it there, but it's not in the PDF.

Any clues?
