What are possible reasons NO SUI taxes are being calculated for ANY employees?

SUI Taxes are set up in Tax Area Information (P069012) and Unemployment Insurance Rates (P079221).  However no taxes are calculating for anyone and it is all going to Excludable.  The F07353 shows a Vertex Error Code of NGRS (No Gross Pay).  The Gross Amount is in the Gross Pay (G101) and Excludable (G201).  The Amount/Rate field (RTAM) is also populated.

Any suggestions on troubleshooting are greatly appreciated.  I am working with a client and it seems it has NEVER Worked for this client.
