PeopleSoft ELM - Learning Instructor Homepage - Class Action Tile showing Oracle logo

In PeopleSoft ELM 9.2 (Image 21) and tools 8.59, Instructor when they login and switch to the Homepage to Learning Instructor, 3 tiles are shown Instructor Preferences, View schedule and Class Action Tile. The class action tile always displays number of pending actions which needs to be done by the instructor. In our application its showing Oracle-PeopleSoft Logo instead. Any idea how to debug this issue and what could be the possible cause?
