Oracle forms and report application opens default login form first and my login.fmx after launch

In a fresh install of oracle f&r 12.2.1.4 and weblogic 12c, I configured everything to successfully launch the forms app. All is working fine except when i'm logging into the forms app, i'm presented with a default login screen. After entering my credentials i'm presented with my login.fmx login form where i have to enter the same credentials again. Only after entering my creds for the second time the app loads.

Webutil is working as I can open the webutil_demo form.

What am I missing in order to bypass this default forms login screen so that my login.fmx login form loads first.
