Access Control Permission Issues

Is anyone experiencing permission settings that don't stay set? We are experiencing anomalies where settings are disappearing. Example: we had the permissions wiped out in Vendor Coordinators group, only when viewing the permissions. We looked once to verify what they had, and the boxes were all selected; we went back in to check again later and they were all deselected and had to re-check the again.
