Where would a Middleware (Biztalk, WebMethods, &c) be necessary with an Orch?

In which instances would a Middleware (Biztalk, WebMethods, &c) be necessary in the context of using an Orchestration?

I'm working on a project in which there is a firewall in the middle, & supposedly the Middleware is an easy way to get around it. This, however, complicates designs and developments, as an extra effort needs to happen to map the external system, the middleware and the orchestration.

Any thoughts?
