ora-20004: uninitialized state in install_file

While installing the OCT CPU OJVM Component Database PSU 12.1.0.2.211019 patch 33248785 Windows platform and Oracle 12.1.0.2.211019 WIN DBBP Patch 33174365 windows platform I am getting the following errors "ora-20004: uninitialized state in install_file" I have already installed these patches on 9 other servers and have not received this error. I am not very familiar with oracle and haven't seen any others experience the same issue.
