Urgent Assistance Needed - Medicare Additional Withholding over 200k not calculating

We have an employee for the first time in history that is exceeding the Medicare withholding threshold of 200k. The system is not calculating the excess over 200k at the 2.35% rate. Any suggestions on what may be causing the issue.
