Employee's 401a deduction stopped this pay period but has not reached max limit

Hi Experts,

We have an employee's 401a deduction stop this pay period even though they have not reached their max limit amount. They have their percentage set at 6% however last pay period it also was not deducted correctly.

Any suggestions on how to correct this?
