HR Checklists and Native Electronic Signatures Issue

We are trying to create agreement pages for our annual trainings (I.E., "I have watched this video or read this policy and agree to it") using native electronic signatures in Checklists which works just fine and that part we're able to complete without issue. However, we also need the user to take a quiz which is scored (creating the quiz goes fine and assigning a score to it goes fine as well). Using Assessments to enable a passing grade minimum is where I run into an issue, and it's two-fold:

1) I can't add a questionnaire from the Questionnaires folder to the Checklist Template since it's only looking in the HR Checklists folder.
