OPATCHAUTO-68061: The orchestration engine failed.

In the middle of applying patch with this command

 sudo opatchauto apply <patch_location>/31390370 I got hit by the above error. in this log /u01/app/19.3.0/grid/cfgtoollogs/opatchauto/opatchauto2022-01-12_09-08-04AM.log here is what I found:
