Vouchered and Paid Dates on single form

Hi

Our version of JDE is 9.2

I'm looking for a single form that includes both the Vouchered Date and Payment Date. I have found them on P0411 and P0413M but cannot locate a single form to view both without using ROW exits, changing forms, etc. I also reviewed the power forms.
