Form Corruption issue

We are getting intermittent form error across all the form post PO patch application in the PROD . as an temporary work around , we are recompiling the form ,library plls and the issue goes away however it reappears once again after few days . This issue we are able to reproduce in cloned environment .

FRM-40815: Variable GLOBAL.IGI_PREV_ORG does not exist.

FRM-40815: Variable GLOBAL.IGI_INSTALLED_ORG_VAR does not exist

FRM-40815: Variable GLOBAL.OPM_GML_INSTALLED1 does not exist.

FRM-40815: Variable GLOBAL.ORG_ID does not exist.

Error Message: FRM-40039: Cannot attach library RCVTXECO while opening form RCVTXECO.
