Repvfy upgrade failing with unique constraint (SYSMAN.EM_DIAG_REP_EXT_PK) violated

We have upgraded repvfy many times over the years but came across a failure for the first time today, upgrading from 20.12.02 to 21.12.21.

SR has been raised but wondered if anyone else come across this?

OMS version is 13c Release 4 Platform Update 6

Repository database is 12.1.0.2.0
