Freight Adjustments are Duplicating in my GL

Every now and then when we do a freight adjustment in Freight Audit History, the adjustment amount duplicates in the GL but only show in Freight Audit History once. When adjustment duplicates in the GL, both adjustments have different batch numbers. We have not been able to duplicate this on demand or know how this happens. Do this happen to anyone else?
