Would anyone be willing to share setup documentation on HSA setup?

We are adding an HSA medical plans with our upcoming open enrollment and I was wondering if anyone that has an HSA account would be willing to share set up documentation. We will have both an employee contribution and employer contribution. How do you handle the age 55+ catch-up provision? Contribution limit setup?

Any assistance would be greatly appreciated.
