PO - business event oracle.apps.po.event.po_approved is not getting triggered

It works fine when I run in TEST mode , but it's not getting triggered after the PO is approved.

I did update the Phase to be < 100. I have set to 98, so assuming that it should get triggered immediately after the PO is approved. Any suggestion on how to debug it ?
