ACA - 1094C - Correction form checkbox is not checked.

We are on 12.1, and We applied Phase II patch and transmitted the file to IRS, then we ran the corrections, and send a correction file to IRS, and noticed that 1095c correction form check boxes are not checked for corrected employees. Any idea what may cause the issue? I have never see the issue with this one.

Did anybody experience the issue?
