Add acknowledgement in Fluid Self Service Savings Plan page

Hello,

We have a requirement for first time 401(k) participant to accept the plan acknowledgement before they can contribute to the savings plan. Have anyone had experience adding an acknowledgement subpage in their self service savings plan page?
